| Provides In-Home Aide Services/personal care service and/or chore service to frail elderly and adults with disabilities to help them remain in their own homes. An assessment is required to determine eligibility for the program. Eligible clients must agree to pay any fee required on a sliding fee scale. In addition, emergency temporary back up coverage for personal care services is available for residents who use the Medical Assistance Personal Care Program. Call if you want an assessment. |

For persons who are between the ages of 18 - 64, have a severe, chronic or permanent physical disability and need help with activities of daily living, self-care, and mobility.

| The Maryland Department of Health and Mental Hygiene administers the Maryland Community Choice or Living at Home Waiver that provides services for those eligible individuals that meet a nursing home level of care and are between the ages of 21 and 59 years of age. This waiver is designed to provide consumer-directed personal assistance services for adults with physical disabilities in their own home. Waiver services include attendant care, administrative case management, family and/or consumer training, skilled nursing supervision, personal emergency response systems, environmental accessibility adaptations, and occupational and speech/language therapies. Persons receiving Medicaid and living for at least 30 days in a long term care facility are automatically eligible for the Living at Home Waiver. If the person receives both Mecicaid and Medicare the wait is at least 60 days before being able to apply for waiver services. |

For older adults who are age 50 or over. Provides services that are typically covered by Medicaid only in long term care facilities to be provided to eligible persons in their own homes or in assisted living facilities.

| The Rare and Expensive Case Management Program (REM) is a Maryland Medicaid program. It is part of Maryland's managed care system known as HealthChoice, and it is administered through the Center for Health Policy Development and Management (CHPDM) located at the University of Maryland Baltimore County. The program is intended to provide specialized services to people with special health care needs who are also eligible for Maryland Medical Assistance. To be eligible, the person must have a diagnosis that is included in the program's criteria. The individual's special needs must be considered to be outside the scope of services provided through Medicaid managed care organizations. |

Provides personal care to people who are 65 years old or older, have a disability and are at risk of entering a nursing home, your income is not more than 60% of the State's median income per month, and your assets meet the eligibility requirements.

| Visiting Nurse Association (VNA) provides short-term, skilled home health care to medically needy Montgomery County residents of all ages. Third-party reimbursement (Medicare, Medicaid, private insurance); all other fees are based on a sliding scale or waived. | |
