| The Maryland State Department of Education administers the autism waiver, targeted to children ages 1 through the end of the semester that the child turns 21. As a part of determining eligibility, a child must be diagnosed with Autism Spectrum Disorder, meet an Intermediate Care Facility for the Mentally Retarded Level of Care, not be enrolled in any other waiver, and have an Individualized Educational Program (IEP) or Individualized Family Service Plan (IFSP) and receive at least 15 hours of special education and related services per week. The services provided under the Autism Waiver are respite care, family training, environmental accessibility adaptations, supported employment, day habilitation, residential habilitation, and targeted case management. |

| The Medicaid Home and Community-Based Services Waiver for Individuals with a Disability provides services for developmentally disabled individuals, who meet an Intermediate Care Facility for the Mentally Retarded (ICF-MR) level of care, as an alternative to institutionliazation in an ICF-MR. Covered services under the waiver include day habilitation, residential option services, respite care, services coordination, environmental modifications, assistive technology and adaptive equipment. | The Maryland Department of Health and Mental Hygiene administers the Maryland Community Choice or Living at Home Waiver that provides services for those eligible individuals that meet a nursing home level of care and are between the ages of 21 and 59 years of age. This waiver is designed to provide consumer-directed personal assistance services for adults with physical disabilities in their own home. Waiver services include attendant care, administrative case management, family and/or consumer training, skilled nursing supervision, personal emergency response systems, environmental accessibility adaptations, and occupational and speech/language therapies. Persons receiving Medicaid and living for at least 30 days in a long term care facility are automatically eligible for the Living at Home Waiver. If the person receives both Mecicaid and Medicare the wait is at least 60 days before being able to apply for waiver services. |

| This waiver targets medically fragile individuals including technology dependent individuals who, before the age of 22, would otherwise be hospitalized and are certified as needing hospital or nursing home level of care. Through the waiver, services are provided to enable medically fragile children to live and be care for at home rather than in a hospital. Model Waiver services includes case management, private duty nursing, shift home health aide assistance, physician participation in the Plan of Care development and durable medical equipment and supplies. Individuals that would like to apply for Model Waiver Services must contact the Coordinating Center for Home and Community Care, Inc., a case management organization. |

Persons eligible for this program are at least 50 years old, have a monthly income of no more than $1,911, assets that are no more than $2,000 to $2,500, and must meet medical eligibility requirements for a nursing home. Services may be provided in the home or in an assisted living facility. These services include Personal care, Respite care, Home-delivered meals, Environmental Assessment Family or consumer training, Personal response system, etc.
A list of individuals interested in receiving waiver services is being maintained. To place a name on the Waiver Services Registry call 1-866-417-3480. Registrants will be notified as openings become available.
Waiver services are available immediately for eligible nursing home residents, (wishing to leave the nursing home), who stay has been paid by Long Term Care Medicaid for 30 days or more. |
Rare and Expensive Case Management REM Department of Health and Mental Hygiene |

| The REM Program is a Maryland Medicaid program that is part of the Maryland's managed care system known as Health Choice. It is intended to provide specialized services to people with significant health care needs who are also eligible for Maryland Medical Assistance. To be eligible the person must have a diagnosis that is included int he program's criteria. The individual's special needs must be considered to be outside the scope of services provided through Medicaid managed care organizations. |

| Traumatic Brain Injury - Adults: |

For residents age 22 through 64 who have been diagnosed with traumatic brain injury and meet the following criteria:
Individuals with traumatic brain injury who require a specialty hospital or nursing facility level of care.
Individuals must have experienced the (initial) traumatic brain injury after the age of 21.
Individuals must be inpatients in a State Mental Hygiene Administration facility, inpatients in a state owned and operated nursing facility, or individuals who have been placed by Medicaid in an out-of-state facility.
Monthly income of no more than $1,809.
