To obtain a disability parking sign for a non-residential building, please put your request in writing and mail or fax to the Division of Operations. This department is responsible for installing and maintaining traffic signs and pavement markings, overseeing the installation and maintenance of streetlights, managing neighborhood traffic calming, roadway safety investigations and data collection programs, and overseeing the occupancy of our public right of way to ensure that work zone activities and special events occur with appropriate traffic controls.

Driver's licenses, disability tags, identification cards and residential street parking signs. To obtain a disability license plate or placard:

  • Check with your doctor as his/her office may have the required application/certification forms.
  • Visit a senior center.
  • Visit any Motor Vehicle Administration (MVA) office to pick up an application/certification form for a Disability Parking Placard/Tag or click here to download the form.
  • Call 1-800-950 1MVA.

Your doctor must sign the application confirming that you need the use of a disability placard/tag either temporarily or permanently. After your doctor has completed the application/certification, bring it to a Motor Vehicle Administration (MVA) office where a placard or hard tag will be issued.
