Birth and Death Certificates

(Vital Records)

The Montgomery County Department of Health and Human Services issues birth certificates for anyone born in the State of Maryland and certified death certificates for 30 days from the date of death for individuals who died in Maryland.

Birth certificates are available for any individual born in the State of Maryland.  To obtain a birth certificate at the Montgomery County Office of Vital Records, the applicant must have been born in Maryland AFTER 1939.  Those individuals born BEFORE 1939 should contact the Maryland Department of Health and Mental Hygiene's Division of Vital Records at 410-764-3063.  Individuals seeking a birth certificate must present  valid, unexpired, government-issued identification such as a driver's license or U.S. passport.  The identification must have an issue date and an expiration date.  To download an application, click here (English) (Spanish).

Walk-in hours for ermits for exhumations (disinternment) are issued with the assistance of a licensed funeral home. Birth certificates are NOT issued in Montgomery County.  Birth certificates for individuals born in the State of Maryland are issued through the  Maryland State Division of Vital Records  or by calling 410-764-3036.  

Walk-in hours to apply for death certificates are Monday through Friday from 8:30 to 11:00 a.m. at the  Dennis Avenue Health Center

Bring completed application or provide the following information:  decedent's name, date and place of death, funeral home name, relationship to the deceased and applicant's full name.  A valid government issued identification (such as driver's license or passport) is required.

  • Fees for death certificates are $32 for the first death certificate and $20 for each additional copy.

Death certificates can also be obtained by mailing a copy of the death certificate application and copies of required identification, along with fees, to the Division of Vital Records, 6550 Reistertown Road, Baltimore, MD  21215.
