Maryland Advance Directive: Planning For Future Health Care Decisions
The Office of the Attorney General has a publication (which includes forms) that you can use to plan for future health care decisions (PDF). The form is optional; other forms are also legally valid. If you have any legal questions about your personal situation, you should consult your own lawyer. If you decide to make an advance directive, be sure to talk about it with those close to you.
