Coordination of Community Services

Montgomery County DHHS Coordination of Community Services (CCS) Program assists and supports individuals with that have been determined eligible by the Developmental Disabilities Administration (DDA) in obtaining the best quality and most appropriate services and supports in their community, to fully participate in their communities, and to enjoy lives that include their dreams and preferences. Coordinators provide case management and advocacy for the individual and/or family to ensure that the individuals health/safety needs are met.
 
Note – Montgomery County DHHS Coordination of Community Services has a cap of 500 clients so we are not accepting any new transfers or clients.  The program currently has a future client waiting list of over 300 individuals
