Camp Forms
General camp information will be emailed to parents prior to the beginning of the camp. All campers must download and complete the Camper Health and Information Forms and Camper Code of Conduct. If applicable, campers should complete the State of Maryland Immunization Certification, Authorization for Medication, Asthma Action Plan Form or the State of Maryland Diabetes Medical Management Plan. All forms must be turned in to the camp director on the first day of camp and each session thereafter.
Camper Health and Information Form
Please complete form and bring to camp on the first day of the summer program. A separate form is needed if camper is registered in multiple programs throughout the summer.

If your child must receive medication at camp, you must send by email [email protected] a copy of a completed Authorization for Medication Form at least fifteen (15) days prior to your child's session.

If your child has asthma and will receive medication at camp, you must send by email [email protected] a copy of the completed Asthma Action Plan Form at least fifteen (15) days prior to your child’s session.
State of Maryland Diabetes Medical Management Plan
If your child has diabetes and will receive medication at camp, you must send by email [email protected] a copy of the completed State of Maryland Diabetes Medical Management Plan at least fifteen (15) days prior to your child’s session.
