Environmental Health Survey Permit & Inspection Process
An Environmental Health Survey is an inspection requested by the Department of Health & Human Services and the Office of Child Care for facilities served by private wells and/or septic systems as part of their annual license renewal. These facilities include daycare, group homes, assisted living, private schools, food service facilities, etc. The survey ensures private well water is safe for human consumption and private septic systems are adequate and functioning properly.
A non-refundable fee must accompany the application at submittal. Please refer to the current fee regulation for information regarding fees for this permit process.
Inspections are generally scheduled within one to two weeks of submission of a complete application. Processing time may vary due to workload factors.
