FAQ's

1.  Who qualifies for an  AERS evaluation? 
Anyone who is medically fragile, disabled, vulnerable and is at risk of institutionalization.

2. Where is the evaluation conducted?
The evaluation is typically conducted in the client’s home, but it can also take place in a nursing home or hospital. 

3. What is the purpose of the evaluation? 
Services are designed to help individuals who are at risk of institutionalization to remain in their homes. It is also for individuals interested in leaving an institution.

4. Is an AERS evaluation required for persons at risk of nursing home placement? 

  • First, apply for Medical Assistance through the local Department of Social Services in the county where the applicant resides. If the applicant is already in a nursing facility, the county in which the facility is located. This can be done by mail or in person at your local Department of Social Services. Be sure to bring documentation of your income and resources, including unpaid bills or receipts for medical services. Be prepared to send copies of them when the eligibility worker begins to process the application and requests additional information.
  • The applicant must be certified as medically eligible for nursing facility services. If the applicant is already in a nursing facility converting to Medical Assistance or in a hospital prior to being admitted to a facility, the facility or hospital will apply for medical eligibility. If the applicant is in another setting, the applicant or representative must have the applicant’s physician complete the Medical Eligibility Review Form, DHMH 3871 and mail it to Telligen for consideration. A Level I screen for Preadmission Screening and Resident Review (PASRR) must be completed by the requesting facility or Physician and submitted to Telligen.  Once the Level I screen is approved by Telligen, the requestor must contact the Assessments, Evaluation and Review Services (AERS) unit for the Level II evaluation.
