Asian American Health Initiative - Patient Navigator Programs
The Patient Navigators Program assists low-income County residents, who speak limited English, by providing health information and interpretation services so that community members can make informed medical decisions.
Services Provided by the PNP:
The Multilingual Health Information and Referral Telephone Line provides general health information and navigates callers through the extensive network of local resources and services available to County residents.
The Trained Multilingual Medical Interpreters physically attend medical appointments provided by County programs with clients, where they provide face-to-face interpretation and help clients navigate the healthcare system in multiple Asian languages, including but not limited to Chinese, Hindi, Korean, and Vietnamese. Interpreters work to empower patients by providing them with the information they need to understand their diagnosis and treatment options, communicate with doctors, and ask the right questions to get the answers they need.
