Read the information provided. Please note that there are some Service Requests that can only be made by calling 311 due to verification requirements that must be completed at the MC311 Customer Service Center. If a Service Request can be created online, you will see a small box entitled “Create Service Request”. Click on the tab and follow the instructions carefully by providing all of the requested information. Service Requests that do not have all of the required information can often not be processed and must be closed.

After entering the characters in all of the required fields, Click on the “Submit Service Request” button. The submitted request will be automatically routed to the appropriate department/agency who is responsible for fulfilling your request. You will receive an email confirmation providing your Service Request number, and additional information about the service that you requested.
