The Montgomery Cares Clinics are geographically located throughout Montgomery County to provide access for residents. Montgomery County Government does not endorse, warrant, or guarantee the products, services, or information described or offered through the Montgomery Cares clinics. Montgomery County Government assumes no liability or responsibility for the services rendered through the individual clinics, nor the actions of the clinics and its employees.
As of July 1, 2019, the Department of Health and Human Services' (DHHS) Office of Eligibility and Support Services (OESS) will conduct eligibility for the Montgomery Cares program. Residents must visit an OESS location or one of ten participating clinics where staff can assist with eligibility determination, for more information visit Montgomery County DHHS.

The Adult Mental Health Program is an outpatient mental health program which provides services, including individual and group psychotherapy, office-based case management, and psychiatric medication monitoring to low-income residents of Montgomery County who are experiencing serious mental illness and who are unable to access the public mental health system due to lack of public benefits or their immigration status or income.
240-777-1770As of January 1, 2014, Medical Assistance (also known as Medicaid or MA) covers more adults ages 19 to 64, whether they have children or not. To qualify for coverage, you must be under age 65, a U.S.. citizen or qualified immigrant, and be a resident of Maryland. To find out more, please visit Marylandhealthconnection. Medicaid also covers former foster children under age 26 who left the foster care system on their 18th birthday. Former foster children do not have to meet income limits--they qualify because of their status as former foster children.

Class Informaiton: AAHP’s Diabetes Prevention Program (DPP) is a structured lifestyle change program modeled after the CDC’s National Diabetes Prevention Program (NDPP). AAHP's DPP addresses the increasing burden of prediabetes and type 2 diabetes among African Americans in Montgomery County, MD.

HealthChoice Diabetes Prevention Program
If you have prediabetes, the HealthChoice Diabetes Prevention Program lifestyle change program may be for you. It can help you lose weight, become more active, and prevent or delay type 2 diabetes. Your health care provider or your health plan will help you find the right DPP provider for you.
A specially trained lifestyle coach will lead and support you and your group members through this year-long program. You can choose what learning style works for you: online, in-person, distance learning or combination sessions.
