DHHS Mobile Health Clinic Core Services

Services include but are not limited to:

  • Seasonal Immunizations: Flu, etc.
  • Testing: COVID-19, HIV/STD, etc.
  • Preventive Screenings & Referrals: Oral Health, physical health and mental health
  • Exams and Treatment: Physical exams, Dental exams and cleanings, etc.
  • Health Education and Health Fairs on a range of health topics
  • Social Determinants of Health: Food, housing, employment, insurance, financial, and social support insecurities, linkages and referrals to medical, dental and behavioral health homes, etc.

Mission

Montgomery County’s community health improvement process. It is an ongoing effort that brings together County government agencies, County hospital systems, minority health programs/initiatives, advocacy groups, academic institutions, community-based service providers and other stakeholders to achieve optimal health and well-being for all Montgomery County residents.

Mission

Public participation in boards, committees, and commissions contributes to the work of County government and provides a valuable service to the community by presenting the concerns and viewpoints of County residents on a variety of issues.

Babies Born Healthy

Mission

The goal of Babies Born Healthy is to contribute to the reduction in disparities in infant mortality, specifically addressing the gap of excess infant mortality between non-Hispanic Black and non-Hispanic white populations.

Babies Born Healthy Services

The BBH program provides care coordination services to help at-risk pregnant women connect with resources and to help them stay healthy through their pregnancies. Services include prenatal education, and screening for depression, substance abuse, intimate partner violence and environmental safety. Ensures that mom is going to prenatal care and that she is following the advice of her prenatal care provider.

BBH provides intense education on the prevention of preterm labor, the harmful effects of smoking- drugs-and alcohol, parental nutrition, harmful effects of intimate partner violence, safe sleep for newborns, importance of breastfeeding and the importance of family planning and child spacing. Vision of a Healthy Community

The people of Montgomery County believe that a healthy community is a place where people live healthy lifestyles, where there is low crime, safe neighborhoods, good schools, good jobs, access to health care, access to parks and recreations, affordable housing, and strong family life.

Purpose of CHNA

Healthy Montgomery’s 2023 Community Health Needs Assessment (CHNA) identified key health needs, barriers, and issues in the County as shared through the perspectives and reports of more than 2,500 county community residents and stakeholders. This CHNA has findings from state and federal sources that highlight disease burdens across Montgomery County over time. Residents shared their insights into the County’s health disparities through focus groups, key informant interviews, and surveys. This CHNA is a resource for developing evidence-based strategies and best practices for achieving equitable health care for everyone in Montgomery County.
