System Transformation

Montgomery County Continuum of Care and our partners are engaged in an on-going system transformation process, shifting to a person centered, housing focused, low barrier, data-driven homeless crisis response system.  The purpose of this system is to reach our collective goals of reducing disparities and making homelessness rare, brief and non-recurring in Montgomery County. Centering results for the most vulnerable members of our community helps us determine what is working and what gaps still need to be addressed.
